Our team conducted a comprehensive system assessment and gap analysis, applying both qualitative and quantitative methods to evaluate how well home and community-based services are meeting the needs of older adults and individuals with physical disabilities.
We translated these findings into actionable, data-informed recommendations—identifying service gaps, highlighting equity considerations, and outlining opportunities to enhance care coordination and accessibility.
